Liberty Wellness is an out of network service provider which does not bill health insurance companies for service provided. If a person receives rehab service and has out of network health insurance benefits, Liberty Wellness will provide documentation needed to submit to the health insurance company for reimbursement.
Payment is due at the time of service.
